Admission info




So that we might properly assess and appropriately provide professional, comprehensive vocational and community living services, we feel that it is necessary to gather extensive information on each prospective client.

Admission Application


CARAMORE 16 STEP ADMISSION PROCESS

1. An inquiry about the Caramore program is made.

2. An offer is made to applicant and other interested parties to tour the Caramore facilities. The other various opportunities for people to learn about Caramore and its services are discussed, i.e. Caramore’s website.

3. A Caramore application packet is made available to interested individuals. The application packet includes information regarding staff qualifications, program policies, service fees, client rights and responsibilities, and the overall programmatic mission and functions.

4. The Caramore application is filled out, consent for release of information forms completed, and necessary information is gathered together. A copy of a birth certificate (or a passport) and social security card (or a receipt indicating an attempt to reacquire one) will be needed for admission.

5. The Caramore application, release of information and pertinent information are sent to the Caramore office.

6. The Caramore application is reviewed by the Caramore Admissions Director.

7. The Admissions Director sends a potential client’s signed Notification and Release form to obtain the individual’s criminal background.

8. The Admissions Director sends consent for Release of information forms to obtain medical records and reviews medical records once received.

9. Additional services, records and/or further testing may be requested.

10. A personal interview with the Caramore Admissions Director may be required to fully evaluate the applying individual’s appropriateness.

11. A potential funding source is identified. Vocational Rehabilitation (VR) is typically the funding source. Current VR clients should discuss Caramore services with their VR Counselors.

12. A decision to schedule a 5-day program visit is reached. This offers the applicant the opportunity to make an informed decision regarding program admission. The visit also gives Caramore and Vocational Rehabilitation the opportunity to further assess applicant’s appropriateness for admission.

13. The visit is scheduled and occurs. The visit lasts one week; generally, from 8:30am, Monday through 3:00pm, Friday. During the visit, the applicant is interviewed by members of the Caramore Staff and VR, and fully participates in the Caramore Employment Preparation and Group Home programs.

14. An admission decision is reached typically within five working days after the visit, and the applicant is notified of the decision.

15. An admission date is set or the individual’s name is placed on the Caramore waiting list and the individual is notified of his position on the waiting list.

16. If admission is denied, the applicant and/or referral source are to be notified of the programs decision, its rationale and whenever possible, a recommendation for alternative services is given. Due to the nature of mental illness, such notification may be communicated to the client by the person making the referral to Caramore.
